New Patients

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please could provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Patients outside of the Practice Area

Historically, the doctors at Park View have been willing to register patients who live outside of the practice area, or retain patients who move outside of the practice area. However, in recent years we have had to review this situation. In order to provide safe and equitable care to all our patients, doctors must be able to visit in a timely way if a patient becomes housebound, and arrange appropriate community health and social care.

Arrangements to expand or remove practice boundaries are entirely voluntary. Practices are legally entitled to request patients leave the practice if they live outside of the practice area, even if they have been with the practice for some time. If patients do not do so, when requested, they may be removed.
